Specific risks

Increased risk with cervical level injuries that may have:

  • reduced muscle bulk around the scapula
  • poor balance due to issues with trunk control. This can increase shear over the scapula through uncontrolled movement.
  • use of a halo-thoracic brace

The scapula is at the highest risk:

  • in supine or sitting upright
  • with use of poor quality back supports with compressed foam, damaged straps on the shower commode or poor fit
  • with use of an ill-fitting or textured sling is used directly against the skin

Management of breakdown

  • Assess weight and nutrition to address bulk around bony prominences of the scapula.
  • Review posture and backrest supports.
  • Consider offloading the area in sitting with modifications to the backrest or shower commode.
  • Prosthetist and/or orthotist review and assistance with wound care.
