Specific risks
Increased risk with cervical level injuries that may have:
- reduced muscle bulk around the scapula
- poor balance due to issues with trunk control. This can increase shear over the scapula through uncontrolled movement.
- use of a halo-thoracic brace
The scapula is at the highest risk:
- in supine or sitting upright
- with use of poor quality back supports with compressed foam, damaged straps on the shower commode or poor fit
- with use of an ill-fitting or textured sling is used directly against the skin

Management of breakdown
- Assess weight and nutrition to address bulk around bony prominences of the scapula.
- Review posture and backrest supports.
- Consider offloading the area in sitting with modifications to the backrest or shower commode.
- Prosthetist and/or orthotist review and assistance with wound care.
